Stanislaus County submitted a well thought out and robust CPP. The “Design Team” has a dynamic governance structure which clearly powers the cross sector collaboration framework. The CPP details aspects of how you analyzed the data, receive feedback, select target populations, select services and support engagement of all while keeping the main goals of prevention services in focus. Stanislaus County has selected Nurse Family Partnership, Healthy Families America, Homebuilders and Family Check Up as their Title IV-E EBP’s.
